Click on the survey link. The introductory paragraphs say “In this survey, we would like to hear directly from NDIS participants about their own preferences about wellbeing.”

Sadly the survey is only for people on the NDIS, not for all disabled Australians. Missing some important groups (eg 65+ or not sufficiently disabled to be eligible), thus a biased “wellness index” result
