I'm writing a book on this and why I think it's happening. Here are some forces at play that may explain our poor performance with life expectancy since the 1980s.

I'm posting this as a pre-committment device to complete this by the end of the year.
15 years in the making (with the second R&R 5 years in the making - we have no expectations).

There are a lot of policy effects that are larger and quicker than what we would expect (based on what the policy actually does).

Why is it that, at every level of (county) income, #lifeexpectancy is worse in 2019 than in 2010?
This is shocking, and a marks general deterioration of the #social and #economic factors that shape #health.
We need high-quality, rigorous, courageous #science.
To solve the #problem of our time.

From some slides I made on #difference-in-differences:
I came up with a checklist (geared towards people seeing DID for the first time).
I know @pedrosantanna.bsky.social has a great checklist already (which I have fully adopted). This is meant to be an intro to that type of thinking.

Powerful essay and I totally agree. We have to fix the house and better engage others.

Here's the #lifeexpectancy curve -- plotted by income per capita at the state level:
The generalized decline between 2010-2019 implicates broad social, economic, and policy forces, and should move us to look beyond the reductionist (group/sector-specific) explanations we've been pitching thus far.

New medical technologies can have enormous short and long-run social and economic benefits, provided there are opportunities to leverage improved health in labor markets.
Early life access to sulfa drugs improved incomes among Black men. But only where discrimination was relatively low (grey line)

These curves describe the relationship between longevity and income in U.S. states.
Historically these curves have shifted up. In the U.S., the 2019 curve lies *fully below* the 2010. That is shocking.
Whatever we are doing now may only entrench this concerning pattern.

In one short paragraph, Manski succinctly captures the problem introduced by the lack of clarity on what is meant by "equity" in medicine and public health.

Changing #classification schemes present challenging and hitherto under-appreciated identification challenges.

3/ I hope to never feel like I'm great at my job.
In honor of that, I will share my first oil pastel drawing, "Observatory at Haleakala." It's not very good, but it was fun.
