(1/2) The presence of peritubular capillaritis (see arrow Fig. 1) is an important component of microvascular inflammation, along with glomerulitis, in the diagnosis of active antibody-mediated rejection (ABMR).

A case of acute tubular injury with both hemoglobin and myoglobin casts. This patient presented with AKI, hemolytic anemia and CK 8000 after being found down for an unknown period.

3/ Most tophi are also associated with crystalline clefts formed by the deposition of monosodium urate (see arrow). Urate crystals are best seen in alcohol-fixed tissue or in frozen tissue sections, since most crystals dissolve after routine formalin fixation.

This EM image shows powdery, punctate electron-dense deposits along the tubular basement membranes, characteristic of LCDD. These deposits lack the organized fibrillar structure seen in amyloidosis & appear as non-organized granular material.

2/ In contrast, Fig. 2 shows a biopsy from a patient with acute tubular injury in which there is prominent luminal ectasia and loss of brush border PAS staining. Other features seen in acute tubular injury include tubular epithelial cell vacuolization, nucleolar enlargement, and mitotic activity.

1/ What does acute tubular injury (ATI) look like in a kidney biopsy? For comparison, Fig. 1 shows a biopsy with a normal appearing tubulointerstitium. The tubules show a back-to-back configuration, and the proximal tubular PAS-positive brush borders are easily identified.

(1/3) This 40-year-old patient underwent sural nerve biopsy to evaluate for a painful mixed large fiber and small fiber axonal neuropathy. The biopsy showed light microscopic features of a chronic active axonopathic process.
