Some trusts and ICB’s are engaged and are working hard to collectively solve and mitigate risks, but unfortunately not all.

While a large proportion of the national fleet is stationary, acting as surge capacity for ED’s, the simple fact is that they will take longer to respond to all patients. This is one of the biggest challenges to safe & effective care this winter. However the answer may not be in the gift of the NHS

Let’s not loose focus that this should be 15, and while the entire UEC system is under such pressure, we can aim for improvements towards 45, we must continue to strive to return to normality where acceptable is 15. If we baseline extremis then we endanger quality standards

This is an isolated comment and is likely to have significant context behind it. Like you say, could be a particular role, area of an organisation, but being transparent allows organisations opportunity for growth, change and renewal.

I suspect the challenges lie in the governance and cost rather than the efficacy - the use cases are vast and clear. Oh, you could add commissioning arrangements in there too unfortunately !
