2) I do agree that doing these 'just because' and without actually considering the relationships of identitied variable is becoming old hat and a little redundant.

What is the actual problem we need to address? Is there evidence that we aren't identifying clinical deterioration quickly enough AND any evidence that acting sooner would make a difference?
