But the thing I struggle with is that the capacity to pay attention is mediated biologically, so then what for people with mental illnesses of all sorts whose ability to maintain awareness is limited?

The crux of the questions that I struggle with in relation to the dhamma is that the suttas say that the way out of suffering is to be present in such a degree as to see "through" the various fetters of craving and grasping.

Meditation was another thing that I kept seeing and the logic of it made sense. If you struggle placing your attention in any one place for a long period of time (as I very much do), why not practice that a bunch so that in time it might get easier?
