The only other time I did this was in Witcher 3 when Geralt and Kira were trading barbs and I called her a snake and it started a fight to the death, given the context it seemed innocent enough and I didn't expect I'd have to kill her over something inane.

The only exception I have to this rule is when the game does something that is completely out of line with the options presented, two examples come to mind. When exploring dialog options with Wrex in ME1 Ashley killed him out of nowhere, so I save scummed then sent her on the suicide mission.
