A severed head is impaled on a large diagonal spike; it enters through the lower back/neck area and exits through the mouth. Blood and residue coats the spike and is dripping from it. A human heart still connected to the head is dangling from viens/arteries. Russian text appears to the lower right of the head that translates to "can't die".
