Each satellite does digital processing, effectively a small eNodeB. Possibly each pair provides some diversity gain and improved sensitivity on earth to space.

Will it just be a blip whilst surplus capacity is utilised on existing sites, or can this be sufficiently revenue generative to justify additional standalone infrastructure?

Unfortunately the 3GPP use cases are created in the complete absence of commercial input.
