There are unscrupulous surgeons (e.g. spine and shoulder surgeons) I’ve run into in hospitals who would just about use nets to pull people in off the sidewalk. At some point somebody needs to intervene and make sure that surgeries are medically necessary and that providers have good outcomes

I have Medicare-while in theory I can see any specialist without a referral, the orthopod and the cataract surgeon both required a recent referral from primary care. That was true even though I had issues that were obvious to the casual observer.

Population health is thought to be better if “insurance” covers routine and preventative care. That redounds to lower overall costs. Upfront payment requirement for an annual physical, flu shot, etc will reduce preventative care

Two things-need to reduce costs to match other OECD countries (per capita). I’d start with the AMA and the RVU system that sets physician pricing

We don’t need that much incremental taxation. Employers and individuals pay enormous amounts of money for insurance premiums. Just send that to the govt

Mutual of Omaha for Part G supplemental. ($158 for 74 yo). Wellcare for Part D ($0/mo). Wellcare generics are almost free for all but one that I get for $250/90 days less thru Mark. Very happy with claims service, but the Part G premium is borderline affordable for me
