It's the ad Donald Trump posted for his perfume, but the exclamation mark after resist is gone and more text is added. In the photo Dr Jill Biden looks like she is stifling a laugh, because she is watching Donald Trump talk about something. She obviously thinks the things he's saying is laughably stupid. There is a thought bubble coming from Dr Biden saying "Do I laugh or Vomit?" since the look on her face could also be one of covering her disgust.
