just added a new section on RIC lines (rapid infusion catheters) to the IBCC

a RIC line basically involves up-sizing an 18-20G PIV to a 13G PIV using a Seldinger technique

we just got these, so I'd love to hear folks' opinions on them & any pearls/pitfalls #EMIMCC

RIC lines have higher infusion rates (~1200 mL/min), fewer complications and are faster to insert than CVC lines (28 s vs 52 s) — why are we not using them more in rapid infusion resuscitation?

It is critical to understand the physiology behind intraop hypotension:

1. Narrow PP: low stroke volume (preload↓, contractility↓ or afterload↑)

2. Low DBP with preserved PP: fast arterial run-off (SVR↓, HR↓ or aortic compliance↓)

3. Both low SBP + DBP: combined low SV + fast run-off

The serum creatinine level is a concentration, amount per volume.

If you quickly expand plasma volume by 20%, the Cr will decrease by ~20%, saying nothing of GFR.

Same goes for ALT, WBC, CRP, and other values that we trend and sometimes overinterpret small fluctuations of.
