I used to visit a hospital for work that actually had an old letter board in the ER waiting room with the prices for various standard procedures listed. Yes, it was a poor neighborhood. This was like 2006-ish.

it's not a cover charge. You pay a cover charge every time you enter. You pay for Costco once per year.
it's great if there's one nearby and you can afford the $65 cover charge, but neither of those things are even close to universal
