#Neurology never stops amazing me. Had an amazing case of monocular quadrantanopia on initial exam that turned out to be ischemic stroke. Differential based on CT included Temporal Crescent syndrome findings, but I had never seen a case. Here is the link:

Med Myth reminder:

If a patient hasn’t used 75% of prescribed meds in 9 months, it’s not treatment failure or a side effect. Adding more meds won’t help. Investigate non-compliance reasons first.

Every time I see a patient for a second opinion who was recommended to see a psychiatrist for neurological symptoms that clearly have an organic origin, I realize some doctors have a hard time saying, "I don't know".

Asymmetric dystonia, contraction, and hypertrophy are not psychological.

As neurologists, we must be clear with patients: treatments like IVIG are unlikely to work for Functional Neurological Disorders. Offering ineffective options breeds mistrust. If you're unsure, saying 'I don't know' is far better than false hope.

I often wonder if we underutilize intranasal lidocaine as #migraine / #Headache abortive therapy. I personally do not know anyone using it but literature support seems worth trying. Would love to hear if people have different thoughts
