hey how do people like, write, when they're not under immense pressure to do Literally Anything Else (like study). The executive dysfunction sure is dysfunctioning!

That isn't what OP said at all. In terms of getting certain care (mental healthcare, HRT, etc.) doctors often deny this for reasons completely unrelated to actual medicine unless pressed for second opinions/pressed in general. Hence, pushiness is needed.
