This is a remarkable podcast which I’ll admit was an emotional journey to listen to. The people involved, including the inspirational @Susanna Stanford have helped drive change in healthcare around safety, best practice and a woman’s right to be heard.
The @nytimes.com and @serialproductions.bsky.social have released the second series of The Retrievals. It is powerful, compelling listening from Susan Burton with Julie Snyder, Ben Phelan and team.

The 4 episodes are a solutions focused look at intraoperative pain during caesarean delivery.

I love this quote from the report. Absolutely nails the issues I’ve seen in the last 19 years helping to improve healthcare. Getting the culture right is not easy, but it is the key to making a real difference to safety.

Great work from colleagues, the value of “extra help” should be a new perspective (not just extra hands for workload reduction).
Humans are prone to ‘mind lock’ and cognitive bias

Senior colleagues arriving to help tend to follow primary caregiver down wrong and possibly fatal path
