Dural-based mass with dural tail

Negative for SSTR2, no evidence of STAT6 fusion, and strong CD56 staining

GLI1 amplification present

🧠 take home point: GLI1-altered mesenchymal tumors can occur in the CNS and radiologically mimic meningioma!

In honor of upcoming Valentine’s Day here is a classic example of rosette-forming glioneuronal tumor. These grade 1 tumors typically show a hotspot mutation in FGFR1 along with PIK3R1 or PIK3CA alterations and are usually found in the fourth ventricle 🌹 ♥️ 🥀 🧠 🔬 #pathsky #neuropath #medsky

This is a clear cell stromal tumor of the lung. They have YAP1::TFE3 fusions, which are also present in YAP1::TFE3-fused hemangioendotheliomas and some pulmonary PEComas. Abbas Agaimy described these. pubmed.ncbi.nlm.nih.gov/33899786/

Just like Texas rarely sees snow ❄️, every once in a while you will find a primary tumor of the posterior pituitary so keep it in mind during frozen section analysis! Here is a classic example of pituicytoma showing diffuse nuclear positivity for TTF1 #pathsky #neuropath #medsky 🔬 🧠

#PathSky 32M with extra-axial, dural-based, frontoparietal mass. Clearly malignant epithelioid cells arranged in cords and nests in a sclerotic background. Looks a lot like a sclerosing epithelioid fibrosarcoma, and it is strongly MUC4-positive. So, we’re done, right?

Some tumor look-a-likes on this Friday:

Chordoid meningioma and chordoma can share similar features of cords of cells in a myxoid background. Chordoma shows diffuse nuclear Brachyury expression whereas chordoid meningioma is positive for SSTR2 helping to differentiate the two 🧠 #pathsky #neuropath
