A lost book checklist titled "I’VE LOOKED EVERYWHERE!" with a teal border and a background featuring illustrated books and laptops. The form is divided into two sections: "At school..." (Get a teacher’s initials) Checklist items include cleaning out a locker, emptying a backpack, checking classroom shelves, asking teachers about the book, checking the Lost and Found, and speaking to someone about it. "At home..." (Get a parent/guardian’s initials) Checklist items include cleaning a room, looking in the car, checking under the couch, searching in closets, and listing three additional places where the book was searched for. At the bottom, instructions read: "Return this completed sheet to the library to have the lost book cleared from your account. If you find the book later, please return it to the library. Thank you for looking!!" A small note states, "Inspired by Laura Woodruff 2023."
