The US funding decision is impacting the HIV response in many countries.

Honduras: Resource shortages have forced 100 healthcare workers to stop providing lifesaving services, limiting access to HIV care. Community organizations are also struggling due the cuts. Prevention programmes have been disrupted, with PrEP distribution for LGBTQ+ people suspended, increasing the risk of new infections, especially among key populations. Mali: PEPFAR supported HIV projects and the work of civil society organizations were halted. To mitigate the impact and prevent further disruptions to HIV services, the Malian Government allocated 120 million CFA francs to support affected organizations, helping them sustain their activities. South Africa: 15 374 PEPFAR-funded staff and HIV treatment for approximately 222 000 people, including 7445 children have been stopped. The South African government has launched the "Close the Gap" campaign, aiming to enroll an additional 1.1 million people living with HIV on antiretroviral treatment by the end of 2025. Zambia: With US$ 367 million previously committed for 2025 under PEPFAR, the funding shift has led to the closure of 32 drop-in centers. DREAMS centers for adolescent girls and young women in 21 districts have been closed, and 16 voluntary male circumcision centers are closed. 49% of PrEP services are also affected. The halt threatens over 23 000 health personnel, with limited government capacity to absorb them.
