A big thank you to the hosts of our Board meeting earlier this week, United Lincolnshire Teaching Hospitals NHS Trust. We met with their Board and patient safety team to hear about their patient safety approach and priorities, plus share learning.

Last week we held a roundtable with @hssib.bsky.social to discuss how we can reduce racial bias and discrimination in patient safety investigations.

Insights will feed into a joint policy briefing as well as guidance to ensure that investigations are fairer and better support improvements for all.
