As guideline development evolved, a key issue emerged: decisions lacked transparency. The GRADE Evidence to Decision (EtD) framework addressed this by making judgments explicit across key criteria like benefits, harms, and feasibility—promoting clarity, consistency, and updatability. #GRADE25years

The GRADE approach builds on the concept of levels of evidence but offers a more nuanced method to rate certainty by considering not only study design but also key characteristics of the body of evidence.

Decision thresholds are reference values that guide judgments about both the size of the effect (e.g., trivial, small, moderate, or large) and the associated certainty of evidence.
They are one of the most consequential recent innovations.

Twenty-five years ago, the newly formed GRADE Working Group set out to create a rigorous, transparent system for rating evidence and recommendations.
Members even held a friendly contest to name the method, ultimately landing on the now-iconic acronym GRADE

Fifteen years ago, following GRADE was easy—only a few up-to-date papers. Now, the field has grown, and not all papers reflect current guidance.
