They don't. It's not free at the ER for anyone, (unless you have Medicaid and the undocumented can't get Medicaid.)

Government funded hospitals are not allowed to turn anyone away, regardless of ability to pay, but they still get a bill.

What an absolute failure of a human he is. Waste of oxygen.
