The entire IDSA guideline on diabetic foot infection doesn't include the word "cefepime." Not once.

It does recommend piperacillin-tazobactam in certain cases.

I'm not saying you *can't* use cefepime (you absolutely can and should in some cases), but such therapy isn't guideline-directed. 🤷‍♂️

- vax 2 weeks before or 3-6 months after immunocompromise expected
- expect blunted response if given within that window
- defer during acute rejection, illness
- adjust for viral circulation

We largely quit when Biden’s CDC started saying “COVID evolved to be mild” in 2022 and even stopped tracking cases the summer before the election in 2024. whn.global and @peoplescdc.org are actual apolitical public health orgs. Also I still trust medical specialty orgs like IDSA.

Now IDSA (my society👍). Agrees with other liaisons, especially on ongoing mRNA vaccine funding. Points out that mRNA vaccines have been given to more people in a short time than any other vaccine, and the efficacy and safety is well-established.
