#Long-COVID, #Post-COVID und #PASC gehen häufig mit kognitiven Einschränkungen (Cog-LC) einher. Eine Studie aus Japan konnte jetzt zeigen, dass bei #Cog-LC die postsynaptische Dichte von AMPA-Rezeptoren in bestimmten Hirnregionen erhöht ist.

Given the central role of synapses in brain function, investigation of synaptic molecular changes may provide vital insights into Cog-LC pathophysiology. In this study, we used [11C]K-2 PET to characterize the density of AMPA receptors (AMPARs) on the post-synaptic cell surface, which are crucial synaptic components in brain signalling. Statistical parametrical mapping was used to spatially normalize and apply independent t-test for a voxel-based comparison. We selected patients with Cog-LC (n = 30) based on Repeatable Battery for the Assessment of Neuropsychological Status assessed persistent cognitive impairment and healthy controls (n = 80) with no diagnosed neuropsychiatric disorders. The primary objective was to compare [11C]K-2 standardized uptake value ratio with white matter (SUVRWM) as a reference region between patients with Cog-LC and healthy controls, and to define the regional extent of differences. The secondary objective was to examine associations between [11C]K-2 SUVRWM and plasma concentrations of cytokines or chemokines. As an exploratory objective, we tested whether [11C]K-2 PET data could distinguish Cog-LC from healthy controls using a partial least squares based classification algorithm. La densité des AMPAR était significativement élevée dans l'ensemble du cerveau chez les patients atteints de COVID longue cognitive (Cog-LC) par rapport aux témoins sains (HC). L'augmentation systémique de l'expression de l'AMPAR pourrait résulter de mécanismes compensatoires pour la perte inconnue de fonction cérébrale. Cependant, nous avons constaté une corrélation négative significative entre les scores des sous-échelles RBANS, Picture Naming et Figure Recall, et l'absorption de [11C]K-2 dans toutes les régions du cerveau, où l'absorption de [11C]K-2 était plus élevée chez les patients que chez les participants sains. So here’s my evaluation of dealing with an insurance company and having LC (my lawyers will probably prefer this to be deleted).
On one floor, they will deny you for life insurance for LC because you are too big a risk. Across the hall they will deny coverage for cog therapy prescribed …/11

BTW, I now pay for cog therapy at a discounted OOP rate because my current insurance doesn’t cover it at all. Has anyone seen this research? 🧐 LMK 🙏🏻

In a context of no approved Tx for #LongCOVID, what's helped me most is treating comorbidities: Meds for POTS. Meds for ADHD.

Tbc: ADHD was pre-existing & unmedicated - LC compounded it exponentially. Meds make life cost less cog energy overall. Still have cog dysfxn from LC, POTS, covid-induced ME
