For EM and EMS: The person was brought into a medical space, but the medical staff was excluded (vs excluded themselves).
Never cede this space and responsibility to others.

Re: avoiding burnout-
Internal factors like this are essential, but I can’t help mentioning that external factors like having a good paycheck, supportive organization, plenty of time off, etc are also crucial.

(I hope #EMSsky becomes a thing)
Someone asked me recently how, after being in EMS for 12 years I am not crispy, how I kept the important parts of myself.

I’ve been thinking a lot about it.

The best that I’ve come up with is that while everyone will tell you to be harder, the key to surviving is keeping your softness.

-Improved 1st pass success w/ paralytic 1st(PMID: 39425254)
-All about onset of action: Roc 1.2 mg/kg ~ 60 sec. Ketamine/etomidate ~ 30 sec + propofol ~15-20

A peds ED doc once defined status epilepticus for a group of EMS providers as “if the 911 call was for seizure and they’re still seizing when you show up, it’s status.”
And it gives me an opportunity to post this treatment algorithm for Pediatric Status Epilepticus that we put together

On my morning peloton ride and the instructor said something great for new #emimcc trainees/providers: "Be good. But if you can't be good, be careful."
This is how inexperienced folks should be approaching high risk clinical practice: aspire for mastery, but be cautious in moments of inexperience.

Paramedics follow protocols for this, but should also understand the nuance both medically (not all PEA is the same) and in regards to family (explaining clearly in plain speak the what/why/how of the decision to terminate).

It’s a huge responsibility.
How do you all handle determining when to call (as in end) an arrest?

Let’s say PEA, nothing obviously fixable, family unavailable/unprepared to say ‘stop.’

Obviously so much variability depending on situation, but do you have any guiding principles?

Just as an educational point for anyone who wants to pursue a large city/town EMS you need to be prepared to deal and work with local politics. It’s not just medics you need you convince of an important job, it’s the recipients of EMS care (including yourself and fam)
