The fundamental problem stems from clinicians trying to use immunology terms.
Inactive disease is good immunological control.
Active disease is immune mediated liver injury. Not immune clearance or immune active.
We need to pick a conceptual framework and stick with it. I vote immune framework.

Poster 4468 at #TLM24 describes another approach to the #A1AT problem. Use a small molecule chaperone to prevent polymerization of PiZ in the liver. Dosed orally in mice. If it works in humans could fix both liver and lung disease.
