Exceptional question,I should have included that to begin with. This was in about 2016... It seems like we went with Nationwide. Our experience is only one data point, of course!

In my experience, we paid a monthly fee for about 1.5 years in order for the company to designate a false alarm as a "preexisting condition", not cover it, then exempt any gastric issues from coverage for the rest of our cats life. We cancelled and paying out of pocket is cheaper.
