What is the Vancomycin break point for VRSA, VISA and hVISA?

VRSA have high level vancomycin resistance (MIC >8mg/L)

VISA have low leve vancomycin resistance (MIC >2-8 mg/L)

hVISA is susceptible to vancomycin (MIC </= 2mg/L) but a minority of cells (1 in a million) have Vancomycin MIC >2mg/L.
