Early warning scores in maternal sepsis
Detailed evaluation of several EWS in a large WHO database of OB sepsis:
- OB & sepsis specific scores performed better
- none accurate enough for clinical use
- incomplete data is a problem

Is this respiratory acidosis the same?

Limited data!

The acidosis may:
- right shift the fetal Hgb O₂ dissociation curve
- may adversely affect maternal & fetal hemodynamics

Animal studies are not conclusive, and also not applicable to the human situation, given the marked anatomical and physiological differences between human and animal placentas

Also, excess supplemental oxygen may reduce cardiac output (by increasing peripheral resistance) and result in a net decrease in oxygen delivery
November 24, 2024 at 3:07 PM
The fetus manages with very low PaO₂ levels as the fetal hemoglobin is left-shifted
November 24, 2024 at 3:05 PM
Oxygen delivery to the fetus depends on (as for any organ):
- oxygen saturation
- hemoglobin
- blood flow

SaO₂ has the narrowest range of variation, much bigger changes occur in blood flow.
Blood flow likely much more important than small changes in SaO₂
November 24, 2024 at 3:03 PM
We often read and hear that oxygen saturation needs to be kept higher than the non-pregnant patient - is this true?
