PFD reports are now under Reg 28 of the Coroners (Investigations) Regulations 2013. Not sure about summaries, but the reports are all available on the Chief Coroners website as are any responses - searchable by category.

And if a person was discriminated against on the basis they were perceived to have a protected characteristic (eg. they were a man or woman as the case may be) that would be unlawful direct discrimination by perception regardless if they did have that protected characteristic
