So true. I've lost track of the number of post procedure "pneumothorax" calls from alarmed radiologists were simply trapped lungs in chronic effusions. Even worse is when these poor folks subsequently receive chest tubes🤦
the utility of a thoracentesis is inversly related to the duration & stability of the effusion
whenever possible, review prior radiology to assess whether the effusion is acute or chronic #TrappedLung #EMIMCC

my primary concern with telemedicine ICU consultation is diffusion of responsibility:
- primary attending may be falsely reassured by the involvement of an intensivist
- intensivist defers responsibility to the primary attending
