Add file is not working either. It never did in earlier versions but now it's trying and adding folders and file references I can't use

Worst one is that the agent is inserting in wrong files. I now have to be on the specific file for it to work. Undo is not helping as it seems to undo whatever was made last and not related to file/code it suggested, which is confusing and often irrelevant. Bug report didn't work either 😅
