Interesting case of AL-amyloidosis, kappa type. IF showed clear kappa restriction(pictured); amyloid A IHC was very strong but mass spec confirmed AL-k amyloid (patient has a k-plasma cell dyscrasia). Never seen so much non-specific AA staining before. Mass spec FTW. #renalpath #pathsky #nephsky

Intra-capillary occlusive basophilic polymer "emboli" caught incidentally in the allograft biopsy of a patient with longstanding peripheral vascular disease (PVD) who purportedly had a recent procedure for PVD treatment while on a visit to another country.

1st case on BlueSky -
Acute cryoglobulinemic GN in an adult patient with elevated HCV viral load and positive HCV RNA; Immunofluorescence shows positive cryo staining for IgG, IgM (IgM>IgG), C3, kappa and lambda; EM image shows fragmented cryo within capillary loop with swollen endothelial cell.

Another relatively common finding in lupus nephritis EM evaluation. Cytoplasmic myelin bodies most likely related to hydroxychloroquine effect. In this case present in rare endothelial cell. Some vague laminated structures in tubular lysosomes as well.

CKD, proteinuria, Crohn's disease. KBx w/focally granulomatous tubulointerstitial nephritis and IgAN. Common findings in IBD patients (pubmed.ncbi.nlm.nih.gov/38344742/). Pt on TNFa inhibitor (pubmed.ncbi.nlm.nih.gov/36706240/) also reported IgAN/TIN associations. #renalpath #pathsky #nephrology.
