Haven't seen preterm babies w/ compensatory hypocarbia. Lots of metabolic acidosis w normal lactate + CO2. Interesting topic: clinical assessments often not sophisticated IMO -> unnecessary treatment. Agree re: monitoring for dehydration but hard to weigh accurately early. Does your unit use NIRS?

If BM = probiotics, does that mean that breastmilk is needed for maximal effect of probiotics? Original proprems trial had high rates of breastmilk usage. Or does sheer number probiotics CFUs overcome lack of HMOs? 🤔
