My takeaway was that even if everyone performed well, we've built a system where best outcomes are just a coin toss.

I honestly don't think it was about having morons in charge. With the (possible) exceptions of SecDef and POTUS, everyone did everything right, and the results were still terrible. And no matter what SecDef or POTUS decided, Chicago was gone (or it wasn't, but only because the warhead failed)
