I take my hat off to Stephenson, I really do, but this used to be just how you did. E.g., Richardson wrote Pamela at an astonishing pace with a quill pen. Then a bunch of poor saps typeset it by hand.
And if you're already quite mad, like Neal Stephenson, you might write upwards of 3500 pages with a fountain pen.
