Yes, I build my dungeons a set of rooms at the time to make more interesting and coherent dungeons. I combine that with adversary rosters (google Alexandrian) and random elements like traps and secret doors.

I usually include traps in the random encounter table, and then check when entering an appropriate area.
If I decide that a trap is part of the current encounter I design the monster and trap together, and split the battlefield into parts, and randomly decide if the trap is where my characters walk.

It is a deceptively tricky question. The easy part is that you just ask your oracle of choice wether you've just walked into the trap, then resolve the detection/disarming the usual way. The harder part is deciding when to ask the oracle about a trap, and there are a lot of options here.

Add in option for random encounters to be locations and/or include waypoints and it works very well in my experience.

Just spend the points and check if anything is found. In my campaign I treat every hex as its own territory, so I speed things up by checking on certain thresholds without expending points.
