You make a checksum of the OS image. Then, prior to boot, you make another checksum and compare the two. If they don't match, your OS is at best corrupt; at worst tampered with.

Fellow havers of #ADHD, these pill bottles have a timer on the cap that tells you the last time you opened the bottle. I haven't accidentally doubled my meds in ages. I still completely forget sometimes, but I try to leave them in my eye-line.
