That can happen in any jail by any law enforcement agency. (And does happen). The responsibility rests with the person to provide accurate identification to law enforcement. Verification is a process that takes some time, even longer if you don't have any priors.

Purposely misleading in that the reader assumes officers knew he was 15 when they placed him there, and that they purposely allowed him to be housed with adults.

It says in the article that he lied about his age (and likely other identity information) to law enforcement. It was his own dumb fault for ending up there.
