5) Why do some transplant recipients with obesity not engage in lifestyle changes?
😨 Fear that activity may hurt the allograft or low self-esteem, as shown by @verner-venegas.bsky.social

4/ How does obesity affect recipients?
⚡Delayed wound healing
⚡Increased risk of CVD death post-transplant due to the metabolic complications
⚡Delayed graft function, as pictured in this VA by Dr. PS Vali.

3/ Given these risks, how effective are lifestyle interventions for donors?
💡A study finds only 13% of donors with obesity are able to lose weight and donate.
💡There is increasing interest in the use of weight loss medications, especially the new GLP1 inhibitors, in achieving target goals.

2a/ How does obesity affect donors?
💡Obesity affects the pool of available donors, with many centers using a BMI cut-off of 35kg/m2.
💡Donors with obesity have higher risk of reaching ESKD as compared to those who do not, illustrated by @edgarvlermamd.bsky.social

1/ Prevalence
👉Obesity rates are 40% in adults (defined as a BMI >= 30kg/m2) and 20% in pediatrics (defined as a BMI >= 95%tile for age and sex).
👉Approximately 30% of transplant recipients are obese.
👉Many recipients gain more than 10% of body weight during first-year post-transplant.

⚡Weight loss is maintained for 120 days past the first dose without repeated dosing (previous panel B), whereas current medications (semaglutide) sees weight regain within 1-2 months.
⚡With more on the market, hopeful these medications will be more accessible.
