To everyone in a similar scenario: the tactic my doctor's office has taught me is to ask, in writing, for:
1) the name, board specialty, and license number of the doctor making the determination the treatment was not medically necessary;

I remember my last time hanging out with Justin. We went on a group trip to an amusement park and there was a sudden storm that closed most of the rides and he let 4 of us with no rain gear huddle together under the water resistant cape he had won earlier in the day to try to stay dry. (2/2)
