Yep. I’ve also seen it a fair few times with spiro - on occasions <125 mmol/L.

Paired osmolalities in a couple of stable pts w mild hyponatraemia on spiro suggest it’s due to increased ADH.

Our research recommendations ….

Should we have included an RCT or intensive vs. relaxed BP goal in dialysis patients?

If yes, what BP measurement and what BP goals? What about patients on PD?

BID study provides some insights into how challenging this could be to deliver!

Our @ukkidney.org guidelines here in BMC Nephrology. 💯% agree that in-unit BP should ONLY be used for determining safety of individual HD sessions and NOT long-term CV risk modification. But changing practice will be a long haul.

… the tricky stuff is not so much about arriving at a diagnosis or treatment plan, but more about helping other human beings negotiate decisions at difficult times in their lives and, as you say, this involves nuance, empathy, patience and care. Do our patients really want an AI chatbot? 2/2

Couldn’t agree more @drstevetaylor.bsky.social … for sure AI speeds up radiotherapy planning (example provided by onc other half), and no doubt has massive role to play in pattern recognition diagnostics (eg radiology, histopathology etc) but when I think what I do day-to-day 1/2
