I think this is important - our documentation shouldn't have any surprises in it. We should be discussing what we think and our decision making process face to face to come up with the best plan. Also important is remembering that our medical language may not mean what we think it does to patients.

Great point - they touched on this! It's important to not just think of the provider's perspective but also how the reader may interpret what is written. Adding more rationale for why certain words/phrases are used can help (and of course, actually discussing during the visit - no surprises!)
