We thank Denning and Kosmidis for reading our review on nontuberculous pulmonary disease (NTM-PD) and bringing attention to chronic pulmonary aspergillosis (CPA) [cite Denning and Kosmidis. Letter to the Editor]. Indeed, it is not uncommon for patients with NTM-PD to have co-infections with other pathogens, such as Pseudomonas aeruginosa, and/or Aspergillus species. We agree that CPA is an important disease that can co-occur with NTM-PD, increasing management complexity and portending poorer outcomes.

In a commercial claims database analysis, <0.5% of patients with inflammatory bowel disease or rheumatoid arthritis developed an invasive fungal infection (IFI) within 1 year of initiating tumor necrosis factor–alpha therapy. Histoplasmosis was the most common IFI type. Overall IFI incidence varied based on region, underlying conditions, and use of certain immunosuppressive medications.
