I feel particularly proud that these guidelines are structured around Te Whare Takatāpui - i really think it's the future of how we think of rainbow & takatāpui health in Aotearoa. And the artwork River Jayden did for us is stunning!

No matter what the politicians say, this is discrimination. Look at how the regulation is worded: This is not a ban on its use for a particular indication, it's a ban for a population.

Rainbow Greens commit to reversing the ban.
The Rainbow Greens network would like to first of all, acknowledge the hurt that the events of the last few days have caused. The recently announced puberty blocker ban is a right wing attack on the rights of trans rangatahi, and it being announced on the eve of Trans Day of Remembrance feels like salt in our wounds. This unjust withdrawal of healthcare from our most vulnerable is an egregious overreach on the part of a government we had already come to expect the worst from, and we condemn it in the strongest possible terms. As we can personally attest, gender-affirming healthcare saves lives. As pointed out by our own MPs, Ricardo Menendez-March and Marama Davidson, this is a decision not even supported by the Ministry of Health brief the government commissioned on the subject. This is part of an imported culture war that does real harm to our rangatahi, and Aotearoa health policy should not be determined by a discredited report commissioned in the UK that contradicts best practice. We are here with you, we feel your pain. Many of us are trans ourselves, and even the rest of us know young people who this decision will affect directly. Make no mistake, we have been doing, and will continue to do everything in our power to fight for a strong party response to this. We deeply understand the importance that trans people across the motu see that they are not forgotten. We will fight for a promise to roll back this decision and a commitment that further withdrawals of healthcare for trans people will be off the table should the Greens re-enter government next year. We tautoko the statements of PATHA, of the Rainbow Support Collective, and of the many other rainbow organisations who have spoken out against this ban. Trans rights are human rights, and human rights are non-negotiable.

AusPATH Statement in response to New Zealand Government's decision to restrict access to puberty blockers AusPATH stands with PATHA, our colleagues in Aotearoa New Zealand, and the trans young people, families, and professionals affected by the recent New Zealand Government's decision to restrict access to puberty blockers. As Australia's peak body for gender affirming healthcare professionals, we are deeply concerned that this decision disregards clinical expertise, research evidence, and the lived experience of the communities most affected. When governments depart from evidence-based practice and sideline the advice of healthcare professionals, the consequences are predictable: poorer health outcomes, reduced help-seeking, and a loss of trust in health systems. Decisions of this kind place unnecessary strain on families who are left carrying the full burden of supporting a distressed young person without access to appropriate, timely care. Puberty blockers are a well-established medical intervention used internationally for a range of paediatric conditions. In Australia, they are commonly prescribed (and PBS-subsidised) for many indications. Removing access to puberty blockers does not stop trans young people seeking the care they require. Instead, it forces families into more difficult decisions, often requiring them to move directly to other gender affirming hormones without the time-buying, stabilising role puberty blockers provide. Families who delay or find themselves unable to access care are frequently left to manage escalating distress, often over the high-risk developmental period around puberty. This carries well-documented risks of anxiety, depression and suicidality, and increases the chance of inter-family conflict and family breakdown. International experience demonstrates that restrictive approaches increase pressure on emergency departments, Child and Adolescent Mental Health Units, and crisis services. They also drive families toward online prescribing routes, importing common pharmaceuticals that would otherwise be safely managed within a well-regulated health system. These patterns are already visible in the United Kingdom following its own ban. The UK Government has gone so far as to require clinicians within the NHS to report parents who support their children's affirmed gender, and in some cases, has applied laws that permit the arrest of parents if gender affirming medicines are found at home. This is not an approach any country should emulate. Claims that the UK is leading a global shift are incorrect. The UK remains the only country with an indefinite ban. While some US states have imposed prohibitions, countries often cited as comparators (e.g., Finland, Sweden, and Norway) have not banned puberty blockers. Instead, they have reaffirmed multidisciplinary team-based care models consistent with those used in Australia. Within Australia, Queensland is the only jurisdiction that has restricted publicly available gender affirming hormones. AusPATH also cautions that reducing access to medically indicated care increases the risk of preventable harm. Over the past five years, government bodies in both New South Wales and Queensland have conducted formal enquiries into state-based gender services. In each case, the reviews led to expansion of services, greater resourcing, and recognition of the risks created when gender affirming care is delayed or inaccessible. The recent events in Queensland and the path New Zealand is now undertaking by restricting access to puberty blockers creates avoidable and unacceptable risks for young people. It also risks replicating the failures witnessed elsewhere when governments override evidence-based clinical care. AusPATH urges the New Zealand Government to reconsider this decision. We encourage all governments to follow the evidence, listen to healthcare professionals who specialise in this area, and centre the voices of trans people, their families, and those with lived experience.

The Professional Association for Transgender Health Aotearoa (PATHA) condemns today's decision from Minister of Health Simeon Brown introducing a ban on new prescriptions of GnRH analogues (puberty blockers) for transgender children and young people who are not currently on them. This ideologically driven ban will have a devastating impact on the lives and wellbeing of our transgender and gender diverse young people. We know that children, young people and their whanau will be highly distressed by this decision. We also acknowledge health professionals' frustration at the way their expertise has been ignored and disregarded. "The ban will lead to a deterioration in mental health, increased risk of suicidality and increased dysphoria in gender diverse children and young people, and will put them at a higher risk of experiencing marginalisation and discrimination," says Dr Elizabeth McElrea, GP specialist in gender affirming care and PATHA Vice-President. "The prescribing of puberty blockers is always undertaken with the utmost care and consideration. They have been prescribed safely for decades for transgender children and banning their use will lead to profound distress in this already vulnerable group." This decision is not supported by the Ministry of Health's November 2024 evidence brief on this care. While the evidence brief identified some limitations in the academic quality of evidence regarding the risks and benefits of GRH analogues, limitations in evidence quality are common across a number of areas of medicine, especially within paediatric medicine, and do not justify a ban on access. "PATHA supports evidence-based medicine, which consists of three key components: academic evidence, clinical experience, and patient preferences and values," says Dr McElrea. "When the quality of available academic evidence is of lower quality, it's incredibly common for clinicians to lean on guidelines drawn from clinical experience." "Practising medicine, and paediatric medicine (not just within gender affirming healthcare), often necessitates providing care without a high-quality evidence base. The absence of high-quality clinical evidence does not mean providing this care is wrong." "Children benefit from innovative medical treatments that improve their survival and quality of life. Pediatric care would all but cease if physicians denied treatments for which the evidence base is imperfect." (McNamara et al., 2024, p. 15) 'An Evidence-Based Critique of "The Cass Review" on Gender-affirming Care for Adolescent The new restrictions will be in place until the conclusion of clinical trials in the United Kingdom, which are not due until at least 2031. "In no other area of care would life-saving medication be withheld from a vulnerable population for six years. This will have a traumatic impact on a generation of transgender children," says Avery Zavoda, PATHA Executive Committee member. PATHA is strongly against the decision. It is a shocking act of discrimination against transgender children and young people. "That New Zealand First announced this news three hours before the Minister of Health shows the way this care has been politicised, and how politics have been prioritised over the wellbeing of New Zealand children," said Zavoda. "Banning puberty blockers will not prevent children from being transgender, despite the disinformation New Zealand First are promoting," says Jennifer Shields, PATHA President. "It's obvious that this has been a political decision made to appease a coalition partner based on an imported culture war, rather than a medical recommendation reflecting the expertise of NZ clinicians and researchers who have been working in this field for over 25 years." "We also call on Labour, the Green Party, and Te Paati Mãori to commit to reversing this harmful decision if they are elected into government next year. It's going to be a critical three years for so many of us and our communities and our causes. The day after advance voting started, researchers in Antartica declared an emergency as sea ice hit a new record low. Time is running out - I don't say that to cause fear, and I don't say that to suggest we're done - quite the opposite. Time is running out and the incoming government will not do what needs doing to prevent the impacts of climate change. They may well look to adaptation, but to do so is to condemn our whānau across the Pacific to losing their homes - it's not good enough. The next three years will be characterised by direct action and community care. It will have to be both coordinated and hyper-local as we start to build the community resilience we need.

The briefing referred to an earlier meeting among officials in early April where the option of Sport NZ "no longer providing guidance" was canvassed. "As noted in the meeting, we would advise against removing the Guidance altogether," the document said. It said the Guiding Principles, published in 2022, had been produced in response to "increasing requests from sports organisations for support regarding transgender participants". The officials stressed it was a challenging area for sports organisations to address and so: "it is important that they have guidance around how to approach this." "We have received positive feedback on the usefulness of the existing guidance through this and, therefore, consider that it should remain available to the sector to support them on this matter." Despite that, Mitchell wrote to Sport NZ on 22 July saying he had come to the view the agency should no longer have guiding principles published after considering feedback from his colleagues. He quoted the NZ First coalition agreement which committed to "ensure publicly funded sporting bodies support fair competition that is not compromised by rules relating to gender".
