That is a really low HBV DNA level! I would repeat HBV DNA in the near future to make sure not lab error and go from there. At least there does not appear to be any indication to start antiviral acutely!

Read about hep b surface antigen mutants but have not personally diagnosed. What was the magnitude of the HBV DNA and was the liver enzymes deranged? I presume this patient is not on immunosuppressives that would have predispose to reactivation?
