Yes, patient is sick with fever and hypotension, very uncommon in ´simple ´ peritonitis and presence of air within the cavity ; bad outcome very likely to happen ; save the patient first and also the membrane to restart PD later

I also agree; colonic perforation is likely to explain the whole picture ! IV antibiotics and restart PD later if patient still wanting
