This isn’t collateral damage. It’s necropolitics: a form of governance where the state decides not only how people live, but who is left to die. Not always by direct violence, but by engineering conditions—poverty, delay, denial, unaffordability—under which death becomes inevitable. Care isn’t being mismanaged. It’s being rationed—by design. And the consequences ripple outward. Families collapse under medical debt. Workers quit jobs or go bankrupt trying to keep loved ones alive. Rural hospitals shutter. Entire regions lose their safety net. The economy slows as people can’t afford to stay healthy, let alone productive. And still, those who engineered the collapse float above it all: tax breaks intact, private care secured, shielded from the very suffering they create. Who benefits? The same cast as always. Billionaires. Defense contractors. Insurance giants. Hedge funds ready to monetize collapse. They don’t fear the cuts—they bankroll them. This isn’t a budget fight. It’s a project to dismantle the idea of care itself. To replace shared responsibility with market logic. To decide—quietly, administratively, fatally—whose lives are still worth sustaining, and whose can be forfeited without consequence. So what do we do? We name it. We strip away the euphemisms. This isn’t belt-tightening. It’s state-sanctioned abandonment. It’s not reform. It’s removal. And it must be confronted—not just with policy, but with principle. We fight not only for Medicare, Medicaid, and Social Security, but for the ethic they once embodied: that care is a public good, that dignity is not for sale, that life should not depend on profit. Trump said he wouldn’t cut these programs. He has. And people are dying because of it. But the deeper betrayal is this: he has helped normalize a government that governs not by law or service—but by withdrawal. By letting go. By letting people die. That betrayal must not stand.
