NPH is real, it’s common, and it’s treatable! #NPH #endalz

It is grounded in the Global Functional State Space (GFSS) — a neuroscience-informed model of brain function and dysfunction. That is why we call it StateViewer. It connects clinical care to systems neuroscience, paving the way for new science and new cures. #NeuroAI www.nature.com/articles/s41...

It’s called StateViewer.
A clinical AI tool that reads brain FDG-PET scans and recognizes patterns of 14 different dementia syndromes — like a specialist would. (only nine were published)
But it’s built to augment, not replace, the clinician.

Not all “Alzheimer’s” is the same.
Some patients have language-predominant, visual, or dysexecutive forms — each with different symptoms, prognosis, and response to therapy.
Others seem like AD, but aren’t. (We call these cases LANS.)
Precision matters.

Take DLB (Dementia with Lewy Bodies). #DLB

DLB ≠ AD ≠ Parkinson’s ≠ PSP ≠ NPH ≠ MSA. But it’s easy to mistake.

A proper diagnosis changes everything. Don’t know how to do that?

One of the most striking examples?
🧠 Normal Pressure Hydrocephalus (NPH)
– Gait trouble
– Cognitive slowing
– Incontinence
It mimics other dementias. But it’s reversible.
If you miss the diagnosis, you miss the chance to help. #NPH

Every week in my neurology clinic, I see people lost in the search for a diagnosis.
Sometimes, what’s causing their dementia symptoms is treatable — but was missed for years.
We just published a new AI tool to change that. Here's why it matters🧵

More accurate protein mapping:

I made some edits for people who think behavioral neurology should focus more on proteins then behavior:

Another day as Doc4 quaternary referral after several tertiary evaluations found "some unknown degenerative condition" without doing a simple test for those. FDG-PET:

Even more of Behavioral Neurology explained in one figure of FDG-PET data. Brain metabolism seems to give a key readout of fundamental neurobiology related to all degenerative diseases of the mind:

From the perspective of FDG, one would never mistake NPH (green nodes are patients with NPH) and PSP (maroon nodes). Given that expertise in this distinction is limited and 60 million people have dementia, FDG should be more widely used.

This is still far and above my favorite scientific conference poster photo! We have since been blessed with a third child (now 7 yo), so I have got to update this shot!

Degenerative diseases carve the mind at its joints. Understanding the information contained in this plot of similarity between brain metabolism across degenerative diseases of mind should be a heavily funded priority.

I use investigational AI in my practice right now and it works great! It is based on this information in FDG-PET scans.

So much of Behavioral Neurology’s knowledge can be visualized through a data-driven, self-assembled graph of FDG-PET metabolic patterns across neurodegenerative and non-degenerative clinical diagnoses. 📊✨
