FWIW, with a fresh install I was able to get it down to "Read your browsing history" and "Block content on any page" using the "On Click" method.

Yeah I think something has changed unfortunately. "On click" isn't sufficient and default permissions from web store are now all sites. :/

Just woke up to this happening again today, but "On Click" doesn't seem keep reduced permissions now. Did you figure out a way to keep it w/o world permissions?
